questionnaire is an important part of the education sessions
for you to get the most out of them
If you have NOT received a copy of the LPIself questionnaire, please e-mail us now so that you can receive a copy, complete it, and send it for scoring. The questionnaire takes about 15 minutes to complete. As soon as possible, the completed form should be faxed to Candace Vincent at NACS at 440.775.4769 for scoring. The deadline is Tuesday, October 18th!
Once it has been received, it will be scored, and an individualized assessment put together for you. Only you and the scorer will see your answers and the results. You will receive your profile at one of the education sessions.
